I think there can be a variety of causes. Some of the ones of concern, I think, are high blood pressure, tumor, and toxoplasmosis possibly. We had a 14 year old feral FIV+ kitty who had an eye issue like this and my vet felt it was a tumor, we then found out she had CRF and toxoplasmosis (vet didn't feel eye related to toxo) and she was treated for just under a year, sucessfully, for all until a urinary issue took her quality of life. At first I'd been so scared of an eye tumor, but the eye was never a problem again. I've had other feral cats with uneven pupils where it never seemed to be a problem.
