That depends on how you want MIDI Thru to act. Fixed will always play MIDI through the same channel (and with whatever patch is assigned to that channel in the Instrument List). Smart will play MIDI through the currently-selected channel (so when you're editing an alto sax staff it'll sound like an alto sax, etc.) Direct and Table both require you to change channels manually from the keyboard controller. If you always want to hear your input as the same instrument, choose Fixed; otherwise, choose Smart.
