An under-fermata is often needed in divisi parts or otherwise combined parts. This is covered in Roemer, p. 29 (and given as the ONLY time they should be used). I should think that they're also needed from time to time in complex keyboard notation.

But for them to go in the wrong place by default is, of course, unacceptable.
