If we are taking the "this is not necessary" and "That is nice to have but not necessary" approach, then the whole survey is per se not necessary as Patrick stated already.
10 to 20 questions is EXTREMELY short for a web-based survey. A Web based survey has nothing to do with a paper survey and I still have the feeling people or somehow comparing it to such a survey.
There is no personal history being recorded, so the point is mute.
As I said. I am open. I presented and proposed my questions, I got some answers. If any of you, including you Ben, wants to do it differently, the
1.) jot down your questions
2.) Jot down the answers and how they should be presented
3.) send it to me
4.) it will be looked over, discussed, sent to Max and then put on-line
