One of the three grommets should have a brass strap that completes the 
ground (the grommets are to reduce noise and vibrations).  The motor needs 
to be grounded.  You said that the first position of the switch is high and 
the second is low.  That's not right!  The first is low and the second is 
high.  Try a small test lead with a couple of alligator clips to create a 
good ground to the motor, and I'll bet it starts working right.  You really 
need to get the last grommet bolt repaired.  The motor will move back and 
forth and eventually break one of the two remaining bolts.
