I'm still having problems with my PVR-250 and ivtv. The first time I play a channel or record a program, there is no sound. This is not a mixer or ivtv bug since (a) capturing immediately after booting up gives sound, and (b) if I initialize ivtv (see below) before starting freevo, then I also have sound. Image is ok btw.
Are you using any modprobe options for msp3400? If it doesn't detect your standard correctly then you'll have sound problems. There is a workaround in Freevo to reset some msp3400 inputs / outputs and that may be the reason you get sound after your first capture.
Also, how new / old are your ivtv drivers and firmware?
