just a wild guess, but considering the sound you described i'd suspect the power supply, it would certainly be worth trying. another wild guess would be a hard drive problem, i.e. the motor having a problem and drawing too much power. it would definitely at least be worth while to monitor the power supply voltages during booting and use for a while. since the video slows down it might be a video card problem, i suppose the chips on the vid card might be making the noise. there could be an intermittent open or short on the vid card. these are wild guesses, but what i'd look at.
also of course make sure all the fans are working and check for loose screws bouncing around. i had the power supply in mine replaced, 3 months later i was terrified when i found a loose screw on the logic board, but fortunately it didn't do any damage. i assume you've run mactest pro on it?
