Any third part PCI cards like a USB2.0 card installed at all ?
Depending on the card , they can cause sleep issues but this should have become apparent after you installed it. You can also have a look in your activity monitor to see if anything is still chugging away in the background while the computer is apparently idle.

Never had any luck with sleep on my QS (2002), yes, has a Belkin USB2 card that has been working fine for years. Either the thing could not be woken or else something like this when woken (from a logged out state):

I thought to give sleep another go the other night on the QS but was rather surprised to see the above in the morning after a waking keyboard press. I won't try this again! Had to simply shutdown and restart. I have always either left my QS on for days (the screens sleeping OK) or simply turned off.
