thats the same one month problem i had with the latest SAM installatoin
I removed SAM from my office pc and placed it on a laptop for use in my home 
office where the stream kicks out from to the streaming server.

I took SAM and moved it to an external drive
i took the MySql and Sql Administrator and moved them to the external drive

Brought them home and placed them on the laptop from the external.
Sam installed fine but only after  i went through a month of CAN NO FIND SOURCE 
with SQL dammit

no idea whatwas wrong
i kept downloading new vesions of  SQL
tried to find a good downloadcopy of the old SQL iwas using and its completely 
gone from DEV.

well i got it working but its my honest opinion that SQL 5 isnt really stable 
enough in its most common installation to run like this.

............got no good help on where to get old SQL or how to setup SQL though

and now... im still thinking that SQL database in the laptop is corrupt 
somewhere

  I will look into that. I had a very funky malware/spybot on my machine,
  after that the old service wouldn't load. Used to work, so I uninstalled ver
  3 of MYSQL and installed 5. Says cannot connect or cannot find service. For
  giggles I loaded the same software on another machine and it works. (sam
  4.2.2 & MY SQL 5.0) I didn't change any passwords just the install location
  of MYSQL is different.

I know that doesnt help your situation, but at a minimum you should be 
using MySQL 5.0

More then likely your mysql config file may have been left on the other 
pc under
C:\WINDOWS
Mysql 3 installers stored these files there, so look for my.ini on the 
other pc.
However that may not even help your situation as migration from mysql 3 
-> mysql 5
will undoubtedly have some unexpected issues.

My advice, start fresh and clean, if you need access to old stats from 
mysql 3,
keep it installed on another machine thats not mission critical but 
affords you access
to the data when needed.
