Comment:
We've come up with a work-around:
- in case of multiple diagnoses, only exclude if they're > 4 months
apart.
In many cases, the diagnoses are on the same day, and in a large majority,
they're within a few months of each other.
Sequence number information is still good to have, but we can get by
without it.
