if you use a concrete example, say the diagnosis selector in mdw, then there's a free text

entry box associated with a  prefix searching combo selection box of accepted , built in terms,

then it's just placing a link a link of the free text to the term ( but actually the code associated

with the combo selected term ). So mdw could do it pretty straightforwardly by eliminating

the either / exclusive or only button that  selects between  free text unencoded diagnosis

and coded diagnosis combo selection,  and when the ok button is pushed,  check to

see if a combo selection AND a free text entry has been made , and create a link in their

coding tables (probably will need a second table for free text storage and a link field to

the static coding table).  Or as some people on this might boast, " ..why it's child's play, my

xx year old could do it, in fact he did, blah blah blah .."

This is what we had been doing in Norway, and what I was advocating ever since
arriving in Oz - some 9 years now already
I advocate it because it WORKS. It keeps doctors happy - they can use their
own terms. It grows over time - no big funding needed to create it ad hoc.
