having been a layout artiste in a previous existence (late 70's when computers weren't an option), i would be impressed that anyone doing layout for medical reasons couldn't do a very passable job with open office
i cheat by finalising the image in photoshop, but with practice i can't believe i couldn't do all that and more with the gimp, another free (open source) option
Scribus does do pdf export nicely too though. Also open source & multi platform.

Lots of nice software in the open source world. Scribus is getting some favorable comparisons with Quark and Adobe InDesign
