When careplans & Health assesments were 1st introduced, we had to keep a
paper copy of the form the patient signed.   Due to various people lobbying
the HIC it was changed after 12 months and as long as we indicate patient
gave consent to health assessment / Careplan and you give patient a copy to
keep that they sign, that is all that is required.  Patient need not keep
his signed copy.

It would be good to find out if you guys need to keep a paper copy of 
the GP Management Plan et al.

My big beef about the DoHA/Medicare/other govt. bodies programs 
involving GPs, is that there are just too many of them for any one GP to 
be involved in more than a couple, but they want you all to be doing 
everything, which is crazy.

Not to mention that each one of them is viewed as needing to be a 
complex piece of baroque art by the guys who design them.
