Hi Peter,

In that case we have what is called "market failure", because obviously the 
customers
(i.e. GPs) don't seem to be able to find the service level they desire, the 
barriers for
them to move to a satisfactory provider are too high or they are unprepared to 
pay for it.

The cause(s) of this market failure I will leave as an exercise for the reader.
