I'm seeing the exact same behavior after upgrading to Lion. Notifications sometimes work, sometimes don't. Previews don't work unless I completely kill and restart Growl, then they work until I close the System Preferences, then they stop working again. I sometimes get sound notifications at random long after a notification should have appeared, or maybe even did appear.
