The startup window uses some information from the preferences file, and also may show up if you have a previous version of Growl installed. The easiest thing to do is to
1) Stop Growl 2) Rename the preferences file (com.Growl.GrowlHelperApp.plist in ~/Library/Preferences) 3) Start Growl. 4) It'll present the startup window, go through it. 5) Restart Growl.
