We have seen this but haven't found a reliable way of duplicating
it. Note that we thought WE might possibly be involved, but we've
added more error reporting internally in WE and still haven't gotten
a WE error report so aren't sure WE is involved. We did figure out
what was crashing in comctl32 and it isn't something lvnav would
trigger unless you were pressing one of its keys which you weren't in
this case.

Yes, there are the occasional speech losses; but I have learned to expect

Probably, if one began isolating scripts, certain ones may be causing
given individuals more of a headache.
This may be a necessary tradeoff opening the screen reader to this new
power.
Given that these often will be 3rd party creations, this may be a bit
tricky to deal with at times.
