I downloaded the "Tom Standard" voice last night. I finally
got it to download, but I decided I would probably like the "Tom Premium"
voices better. However, when I try to purchase this one, I get the message
after my credit card info has been submitted that I already have an English
license. I am willing to pay for another voice. Why can't I download it?

You can download the voices and even install them, but without that
authorization file, you cannot make them work.
I am using the Samantha voice at the moment and it is very good, indeed, even
more responsive with Window-eyes than I was expecting. I have to use my USB
headset for window-eyes to see a "sound card" it can interact with, otherwise,
for some reason window-eyes does not see my sound card. :)
