Thanks for your reply Richard,

I seem to have resolved the issue after doing a complete removal and 
re-installation of the l4d dedicated server. It would seem at some point 
between the last time I updated and the most recent patch level, 
something has been corrupted.

Now running 4 left4dead campaigns on the same box and no issues so far :)
