Had it happen once. I was in at a customer after business hours getting a test IPL done. Wouldn't come up. Some bright spark had decided PARMLIB needed UACC(NONE) - with *no* access list. After hours no-one on-site had authority to fix it. Head honcho had to be rung to provide the other half of the "super-user" password. Took a while - good thing Prod hadn't fallen over. I seem to remember muttering something about banks and auditors.
