In neither version of the APPLY statements posted was GROUPEXTEND or even GROUP
specified. Was there a reason not to use GROUPEXTEND? Was there a problem
that led to the use of the various methods of applying the FMID? If you
suspect it is a packaging problem, why not open a case?

Yes, but GEXT is used to choose sysmods to APPLY.
And the list of PTFs to be applied with the function is a result of GEXT. Note: It is possible to use APPLY CHECK before and get the list of PTFs from the output.
However it is still not related to the issue.
It is not relevant how the list of PTFs was created. GEXT is just one of methods, nothing else. It *should be* not relevant whether I APPLY the function with PTFs within single APPLY command or I apply the fmid first and then apply the PTFs. It should not - but *it is*.

Opening PMR is very good advice, however the product is oooooold and no longer supported.
