One thing I have run into at a couple of mainframe sites is they have had so much turnover in tech support and no process for the ex person to train or leave the new person documentation that the current tech people (and same with application folks) - seem to have no idea how things work! This probably applies to all mainframe staff positions. Sites need to have a process in place to save in documentation or spend a couple of weeks with the new person all the systemic information on apps, system software.
It is worth noting that such a documentation effort should not be left to the time when the documentation is needed. If the old staff member does not leave by choice (i.e., is terminated) then it is common practice to escort the staff member off site immediately, thereby rendering said staff member useless for knowledge transfer.
