I used to think I was experiencing this on my OS X iMac, but lately I really think what is happening is that I THINK I am clicking on a live button but in fact the first click simply brings the visible application to the forefront and makes it active. I don't keep a LOT of applications open at the same time, but you only have to have two open for one to be inactive.
