Are there any error messages in the logs? Look for ERR 005; you'll
usually see this before the queue manager shuts itself down due to a
fatal error.

Perhaps this is a good one for the enhancements wishlist.
When QM starts up, it could first do a scan of the log file and remove or
rename any of the offending messages automatically. It could then send a
notification to the server admin (already used for things like number of
messages sent, etc..).
It beats the alternative resulting in a down server.
