Did you restart SMTPD after updating the list?  SMTPD can also write this
list so it could be overwriting your entries.  If you make manual changes
you should restart smtpd after making your change.

For the past several days, I've been re-entering addresses into the access
control list because they've been disappearing.

Yes!  I've been clicking on Save!

I've been maintaining a separate list of the subnets, marking the ones that
I've entered.  Later I notice that we're receiving messages from the same
IPs that I entered to be blocked.

How do I find the cause of the disappearance?
