In the same respect - What would cause known good mail to be filtered into
the JunkMail box randomly?

It won't. Random doesn't come into play here; your tests and actions do.

What tests are they failing? What tests do you use to block E-mail?

We had a problem with both Osirusoft and ORDB when they were active. A good
portion of valid email was being filtered into the JunkMail, but as soon as
they were disabled, we noticed a major difference in the filtering. What
Blacklists are recommended?

We recommend using our default configuration file (at the manual page) and blocking E-mail that fails the WEIGHT20 test (or WEIGHT10 if you want stricter settings, and have a bit of tolerance for occasional false positives).
