Has there been an increase in message volume, spam or otherwise? With a recent increase in spam, we reached the point where we needed to up the Delivery and Retry Threads for the Queue Manager. Not exactly sure why this would affect POP sending, but it did in our case.

We currently use IMAIL v 8.13, with your basic settings, pop, imap, smtp, web cal, but no LDAP or messaging. About every 2-4 days one of our customers, without fail will call into our NOC and ask us "are you having problems with the mail server, it's running very slow". Initally I just kinda brushed it off, but now I am starting to see it as well, Simply sending a text only e-mail can sometimes take 30 seconds... and I am on a t-1 frame to our mail server, so I know a 20k e-mail can fly on that type of pipe. The only resolution for this is to reboot the mail server, which once I do this it runs perfectly fine for another couple days, then walla back to square 1. We did have an issue where I was working on implementing LDAP, but didn't get around to it and LDAP went WILD, so I shut it off and jumped ship there.
