I think that this is true.  I think that the failure is happening
before the boot archive is even being used. 

I need to know more about the context.  So you say that
some boot environments boot, but some don't.  How are
you switching between them?  Are you just selecting
different ones from the grub menu?  But that implies
that you're seeing a grub menu.  But then when
does the "Error 16:    Inconsistent    filesystem structure"
message appear?  After selecting a grub menu entry?
