An SLA between the user and the provider could do all this with diffserv.
You just mark the more expensive packets with a different DSCP, and you will
be charged accordingly, as specified in the SLA. To work in practice this
would probably need its own PHB-group definition (n price levels), and well
implemented support in the end system.
