Go to http://demo.itextsupport.com/xfademo/
Upload your form.
You'll see the data in the form of XML.
You can change the data, or leave it as it is.
Now click Update XFA data.

When I do this with your 3 page document, I end up with 4 pages, not 1.
Also the Reader-enabling is broken (but that's because the demo doesn't 
work in append mode).

The problem with the form is apparently that all options are shown (too 
much data is shown).
I guess this is an example where you need the full Adobe LiveCycle 
solution, or... as you're using iText 5: you probably need a license, so 
you may want to establish a business relationship with iText software. 
In that case, you'll get support and you can avoid the terrible experience.
