Hi again and thanks for your comments. I need fill out that form in C# and i was trying a possible iText-Sharp solution but I couldn't obtain it as you can see. If I need obtain a license for do it, that's no problem and I need some support too.

Now click Update XFA data. When I do this with your 3 page document, I end up with 4 pages, not 1. Also the Reader-enabling is broken (but that's because the demo doesn't work in append mode). The problem with the form is apparently that all options are shown (too much data is shown). I guess this is an example where you need the full Adobe LiveCycle solution, or... as you're using iText 5: you probably need a license, so you may want to establish a business relationship with iText software.
