Why not use the gentoo guide on [1]?

If you have the option to try it under windows it would confirm the card
is not broken.

The above it likely the issue. Gentoo will install the correct firmware
by default via the package pvr-firmware. Delete the old firmware
in /lib/firmware and re-install pvr-firmware.
