I have the same problem, and (AFAICT) the exact same card-- rev D492, tuner type 57. I've tried 0.3.3[kpx], 0.3.4b, various 0.3.2 versions, and I still get the pulsing static.
For comparison's sake, I'm including the dmesg result from the latest attempt I made with 0.3.3x. I think it's identical to yours, except for the ivtv version:
