Using the hanging paragraph feature is one way to go. However, I've found it frustrating sometimes because there is no quick way to double check your work. For this reason I would consider using a table with 2 columns, one titled "initials", and the other titled "response". If you adjust your document so that you have margins of .5 on top, bottom, right and left, and adjust the column width of the initial column to 1 inch or 1.5 inches, that will leave you with enough room to set the "response" column to a width of 5.5 inches or 6 inches. If when you are done sighted readers do not like the look of the lines from the table, then just change your table line settings to none.
