Consider using Excel or a table in Word. There is a command in both of these programs for entering the date and time. I personally like either of these 2 options over creating a form in Word because both the Excel and Word table options allow you to sort the records. So you could sort by who the messages are from, who they are for, by date, or any other field you have.
I must say, I do not know a lot about forms in Word, so there might be some really good reasons for using them over the 2 options I've mentioned.
