I never include USA in my locations, although I might if my family were more recent immigrants. I do add the country on those rare occasions when I find a cousin who emigrated to Canada or Argentina. I have never comprehended the need for the blank comma convention.
This means most of my locations are 3 part, although most of my burial locations are 4 part because I include the name of the cemetery in the location, e.g. Masonic Cem., Eagleville, Harrison Co., Missouri. Any difficulty this may cause is solved, IMO, by a right to left, rather than the default left to right, sort. I rarely use the word "of" for reasons already discussed, although I do sometimes use "probably" in a death or birth location field. I'm sure there are some highly technical reasons why others would be horrified by my methods, but they work for me.
