I am cleaning up my locations list and have some questions.

Format
Country, State/Prov, County, City

I understand this format for Canada and the US. But for England,
Ireland, and Wales my understanding breaks down.

Is it United Kingdom, England, County, City and where do parishes fit
in?  Or is it England, County, City? I'm confused!

Also how do you handle cities or parishes that have changed counties
since the event such as a christening?
