This is exactly how testing works, at least in Victoria. You fill in a form either online or in person and are given a number that is good for 90 days of testing records.

I did my vaccine at my GP, so I didn't have to register. I don't know what the process is if you register online, if you get a number assigned or not.  But people who go to hubs in Victoria can book in advance or walk-up. It's likely people are still having to provide information when they walk-up.

Does anyone in Victoria know different? Perhaps it's just the lag in NSW to set up Hubs. They're way behind us. BUT the issue of AZ is something that must be handled carefully because so far NO ONE has indemnification if something goes wrong. There was an announcement but no follow through (again) according the Makesh Haikewal on QandA on Thursday night. If I were administering to under 60s, I'd want that in writing and Legally binding. So far it's not. Just Scotty making promises (again).

A record of who was vaccinated is needed, but the patient doesn't need to fill in any paperwork for that, as they have already entered their details when they booked. They just need to identify themselves, so the staff can find their record and add details of which batch of vaccine they got.
