The order in which the CPUs are stopped by Linux is invalid - the FFF PSW
should be loaded last. VM therefore does not recognize the completion of
the signal shutdown handling.

There was a bug in Linux 2.4 code that was fixed in November 2003 which
could cause such behavior (see
http://www10.software.ibm.com/developerworks/opensource/linux390/linux-2.4.21-s390-07-june2003.shtml).
Depending on the kernel level you are using this may be the source of your
problem.
