Not true, some big installation too have postfix. I will not consider
exim much.

I would strongly recommend qmail.
Listen, qmail has a very strong philosophy about mailing and if you sit
back and think about it, you will find that the author is right about
most of things. It is much secure and easy to install a server which
does not provide all solutions out-of-box.
e.g. in a normal office, you do not need pop-before-smtp or
smtp-auth, you can just have ip based relaying. So, no need to have
these codes included by the mail server. If you want this functionality
the code is available (third party, but audited).
An analogy would be if I wanted to display logs on my webserver, would I
install apache just for that? No, I will be using some small webserver.
Similarly, I would never use proftpd or wuftpd for simple ftp.

If you are taking on a larger scale then you would need to recompile the
code and patch it. If you cannot do that then you have no business to
run a mail server. Sorry, qmail won't give an out-of-box pack for that.
I rather depend on few hundred thousand lines of securly written and
easy to use program than a big boalty package which has/had security
holes but provides all functionality.

Easy, use squirrelmail, horde, twig etc.
