I remember reviewing/rating a number of questions.  And while I didn't
find them confusing, what I do remember is that the grammar was often
atrociously poor.  This in itself is distracting.  Every question needs
to be reviewed to ensure that:
1.  it is grammatically correct
2.  it is unambiguos, both the question and the answers
3.  the correct answers are correct, and the wrong answers are wrong

And for FITB, that there can be one and only one correct answer and that
it is apparent that that is the case to anyone with the appropriate
experience level, or if there can be two answers, both are accepted as
correct. (I can give examples if anyone is confused by what I mean by my
comments)

I looked at every question I reviewed/rated for these things and made a
number of comments.  I do not know if they were acted upon or not.
