RT is the way to go, like you said it has all the functionality you have
mentioned below.
Setting it up on SUSE 9.2 ( that was some time back ) wasnt that hard
except for the numerous perl modules that are needed ( used CPAN ).

RT probably does the job, as do a host of other bug tracking
systems. How well do they do the job? That's another issue entirely ...
If I had to pick just one and I had a budget for it, I would probably
take FogBUGZ or JIRA. If I needed it to be free at all costs, then I
would take a look at RT or Mantis.
