A few corrections, just to help keep things straight...

The PowerMac 6500 had a 603e processor, not the older non-PowerPC 
68030, which was in computers such as the SE/30 and the LCIII. HFS 
Extended (HFS+) was released with OS 8.1. OS 8.2 was never publicly 
released.

That being said, I found a link which mentioned the problem you are 
having exactly. Apparently, it involves the 5400, 5500, 6400, and 6500.

Some suggestions from the site are:

Try booting to a Disk Tools disk and then mounting the CD and 
installing.

You may be able to force the computer to boot to the CD by holding down 
the C key, and immediately after the Happy Mac pops up, holding down 
the Shift key  to force the CD to load with extensions off.

The writer suggested booting might be more successful if AppleTalk is 
enabled before booting to the CD.
