This is probably one of the primary differences between voice over and the 
windows screen readers. Voice over does not assume you want certain things 
spoken. Does this mean you do a bit more navigation on the mac than you do in 
Windows? Perhaps, but it leaves decisions up to the user which is how it should 
be.
