Note the error stating the driver cannot collect a list of channels .
That's a known problem. D-Link uses 0x18 as regdomain in some of their
cards. Atheros didn't assign this domain with anything useful, so the HAL
complains about being unable to decide which channels might be allowed to
get accessed in that domain.
madwifi-ng has a different but related issue which is explained in #298.

Open Makefile.inc
make
Why did you open Makefile.inc? In order to modify it? If so, what changes
did you apply to that file?

Already tried to get your card into monitor mode without having created
the STA VAP first hand? Have you been on your main console when you've
tried to create the monitor VAP?
