The trouble is that the whole implementation is flawed. Instead of 2
dialogs (first one with which you can disable it, second one which asks if
you want to disable it for all folders) we should have only one dialog, of
course, with 3 radio boxes:
(*) ask me about it next time
( ) don't ask me again in this folder
( ) never ask me again
It's trivial to do, too... Maybe you should file a bug about it.

Oh yes, the whole custom headers thing is a mess. But as this is hardly
the most urgent part I never have time to change it. BTW, Mark, I believe
there are a few bugs about this and it probably makes sense to close all of
them but one.
