Marjorie Roswell did something like this with Lyme disease. I
can't find my reference to her Lyme page but she also maintains a
general GIS resources page at
http://www.research.umbc.edu/~roswell/mipage.html with links to
Medical Geography. 

BTW, that reference to http://hello.to/healthgeo looks like it
might be good (even a section animating maps to map disease
spread), but it hates Netscape. I get some text, but mostly
little square instead of letters!
