Translucency is the “real transparency” you mentioned. I sorta made it
work when it first came out. But it’s not well tested and probably is
already out-dated/broken.

As far as for “fake transparency”, I noticed a bug last year. In FVWM2,
I can have multiple “desktops”. Somehow, only the mrxvt terminals in 
the main desktop are “transparenct”. The terminals on other desktops 
have a solid color background. I debugged a little bit, tried to find an
X event that tells the changing of background on root window. But I could
not identify the event or some other notifications about that. That was
my last effort trying to dig into mrxvt code, again!
