Hi Dana-
This is a long standing issues for smaller facilities without that 24 hour coverage of in-house Pharmacy.  The knee-jerk reaction has been to look at those 3rd party vendors that are providing "order entry"...at a premium price I might add.  I think the able for nursing to enter the orders that they need requires a relatively small training window.

The bigger issue for nursing seems to be - what do we do with the orders we don't use?  Example: physician leaves 6 orders after hours and only 2 are needed for administration before the pharmacy opens.  If we provide a viable solution to enter those 2 orders, what do we do with the other 4?  Typically, nursing will "sign-off" all of the orders at once.  It seems that a solid P&P needs to be developed and/or re-worked to cover this issue as well.

Best of luck - Kevin.

Subject: [MEDITECH-L] BMV without 24 hour pharmacy (Magic 5.4)

Good Morning Everyone,

We are in the beginning stages of BMV and I have a question for those live with BMV and NO 24 hour pharmacy coverage.

  1. How are you getting the meds onto the EMAR when pharmacy is not available?
  2. What is your acknowledgement and chart checking process?

Our process now is the order is faxed to pharmacy for entering in the morning. The meds are handwritten on a MAR and the chart and MAR “noted” by the RN.  When we go live with BMV and EMAR, there will be no paper MAR to write these orders on, and no off hours pharmacy to enter them into the system.

Any thoughts and help would be greatly appreciated.
