Early W123 diesels  do not have a vacuum modulator , start in 2nd and
will kickdown to first when kickdown soleniod is activated and the
hydralics decide its ok to do so.

Some sit in second gear and then downshift when the accelerator is 
moved down.  If they don't, likely the vacuum modulator signal is 
incorrect and the valve needs replacing (or the hose fixed, etc).
